Polyneuropathies are common, with the incidence increasing with older age. The causes of polyneuropathies are diverse and numerous, and it can be challenging for clinicians to determine the etiology of a particular patient's neuropathy. Painful neuropathy has negative impact on physical and mental quality of life compared with painless neuropathy. Keywords: peripheral neuropathy, painful polyneuropathy, neuropathic pain. Peripheral neuropathy is a disease or degenerative state of peripheral nerves in which motor, sensory, or autonomic nerve fibers may be affected. Polyneuropathy is a generalized, roughly symmetric hereditary or acquired disorder affecting peripheral nerves. It is still unclear how and why some patients develop painful and others painless polyneuropathy. Because there is no definitive single or overall composite factor that explains individual pain vulnerability in polyneuropathy, surrogate markers potentially associated with mechanisms for pain can be used. Neuropathic pain is defined as pain that arises as a direct consequence of a lesion or diseases affecting the somatosensory system. One approach is to assess the individual functioning of the somatosensory nervous system (loss of function/gain of function of nociceptive and non-nociceptive sensory modalities) using quantitative sensory testing (QST).
